Integrated relationship tools
An additional area of consideration during the pre-phase service journey was to provide hospice administrators, and liaison’s with tools to track their patient relationships towards conversion. Similar to a sales CRM, liaisons manage relationships with patients, families, and hospital staff in service of moving a terminal patient to hospice. The KPMG team felt missed details about a patient’s needs could be lost in the process, and relationship management tools could promote better continuity of care.
